Resident Support Services
Our
social workers provide advocacy and support for residents. Upon
admission to Broomall each person is assigned a social worker who is
responsible, along with the interdisciplinary team, to identify
emotional and psychosocial needs and create a comprehensive care plan
to address them. The department also maintains a rapport
with outside community health, welfare and social agencies and makes
referrals as appropriate. The resident’s social worker is also the
first point of contact if a concern arises that needs to be brought to
the attention of facility management. When a concern is brought to your
social worker’s attention, they will complete a written concern form
and follow-up with necessary members of the interdisciplinary team to
resolve the resident's concern.
Our Resident Support Team
The social service department is staffed by four social
workers. In addition, the department utilizes a full-time discharge
planner who facilitates the discharge planning process and ensures
necessary home health services and durable medical equipment are in
place prior to discharge. The Discharge Planner also coordinates
patient/family training prior to discharge for such tasks as medication
and insulin administration, safe transferring techniques, respiratory
training, wound treatment and durable medical equipment training. Our
Discharge Planner also assists in arranging patient transportation for
discharge from the facility and contacts the patient once they have
returned to the community to ensure home services are of acceptable
quality.
Spiritual Care Program
Our
Spiritual Care Program was created to meet the spiritual and religious
needs of our residents. Our Spiritual Care Coordinator acts as
the lead liaison and coordinator with the patient’s/family’s community
of faith. The Spiritual Care Coordinator will speak with each newly
admitted patient and conduct an assessment of the individual’s
spiritual and religious needs and advises the patient’s/family’s
clergy, priest, rabbi or other spiritual caregiver of the patient’s
needs and requests. If the patient and family do not have religious
affiliation but wish to receive spiritual or pastoral care, the
Spiritual Care Coordinator provides support in accordance with the
wishes and beliefs expressed by the patient and/or their family.
The Spiritual Care Coordinator also acts as the
point person for religious services being conducted in the facility on
a regular basis. We offer religious services from many denominations
including, Episcopal, Church of God, Roman Catholic Mass and weekly
Eucharistic visits, Seventh Day Adventist, and Jewish. We welcome all
denominations and encourage involvement in our facility from each
resident’s community of faith.
