Long-Term Care at Broomall Rehabilitation and Nursing
Philosophy
The Interdisciplinary team at Broomall Rehabilitation and Nursing
Center is committed to maximizing functional capabilities of each
patient, promoting each person’s highest practical level of
independence. For long-term care patients at Broomall Rehabilitation
and Nursing Center the goal is to help residents get back on their
feet, able to perform as many activities of daily living as soon as
possible. Clinical and rehabilitation services offer improved mobility,
relief from pain and recovery from debilitating illness and injuries.
The Long-Term Rehabilitation Experience
The intent of the Long Term Care program is to provide the most
appropriate service in the least restrictive setting. Residents receive
room and board, supervision, nursing services, transportation,
recreational and social services. Necessary clinical or medical
services not included in the daily rate are arranged for as needed.
Long-term care services include a range of services and supports that
assist individuals with performing activities of daily living (ADLs).
These include, but are not limited to assistance with eating, dressing,
toileting and medication administration.
Shortly after arrival at Broomall Rehabilitation and Nursing Center our
skilled team of clinicians and therapists will conduct a full clinical
assessment of the individual to identify their needs and begin planning
with them for their recovery and return to their highest practical
level of independence. With a firm understanding that each person comes
to us with individual needs, our interdisciplinary team develops a
comprehensive, individualized care plan. By setting carefully designed,
measurable goals, our team strives to monitor progress toward those
goals adjusting the breadth of services to maximize each patient’s
opportunities for a successful recovery
Each long-term care patient’s progress toward their goals is discussed
by the interdisciplinary team as often as necessary, but typically, at
least monthly during regularly scheduled Standards of Care Conferences
as well as quarterly during regularly scheduled Care Conferences. Both
of these review processes are utilized to promote good communication
between the care team and patients and their families. We pride
ourselves on delivering person-centered care, so each resident’s
experience will be different depending on their needs and wishes.
Our Interdisciplinary Team Distinguished
in the community by our successful clinical and rehabilitation
outcomes, Broomall Rehabilitation and Nursing Center’s
interdisciplinary team includes: the patient’s independent
primary care physician, IV Certified nurses, physical therapists,
occupational therapists, speech/language pathologists, respiratory
therapists, a Certified ET/wound care program, social workers,
recreation therapists, restorative nursing specialists and other
clinical specialists. The team provides a full range of services
including: mobility and endurance recovery, communication and cognitive
augmentation, dysphasia management, orthotic fitting and training and
social, emotional and vocational support.
Patients and their families are also integral members of the
interdisciplinary team, working with the team to establish (and
re-establish as often as necessary) realistic goals and treatment
plans. Together, we will coordinate high-quality services that our
long-term care residents, their families and our community need and to
which they have grown accustomed.
