Short-Term Rehabilitation
Philosophy
The rehabilitation team at Broomall Rehabilitation and Nursing Center
is committed to maximizing functional capabilities of each patient,
promoting each person’s highest practical level of independence. For
short-term rehabilitation patients at Broomall Rehabilitation and
Nursing Center the goal is to help them return home, or get “back on
their feet”, able to perform as many activities of daily living as soon
as possible.
The Short-Term Rehabilitation Experience
Shortly after arrival at Broomall Rehabilitation and Nursing Center our
skilled team of clinicians and therapists will conduct a full clinical
assessment of the patient to identify their needs and begin planning
with them for their recovery and safe return home. With a firm
understanding that each person comes to us with individual needs, our
interdisciplinary rehabilitation team develops an individualized
rehabilitation program. By setting carefully designed, measurable
goals, our team successfully helps more than 30 people per month return
back to their homes and many more to get back on their feet following
serious illness or injury.
Each rehabilitation patient’s progress toward their goals is discussed
by the interdisciplinary team on a routine basis and team care
conferences are regularly utilized to promote good communication
between our care team, the patients and their families. A typical
short-term rehabilitation patient may receive one to three hours of
therapy services per day three to six days per week. When the stay with
us gets close to it end, our Discharge Planner makes the transition
back into the community seamless by setting up home health services and
having necessary durable medical equipment delivered right to the home.
Family training is also provided prior to discharge (see Social
Services). We can also coordinate services with other care providers to
prepare rehabilitation candidates for an acute care rehab program,
extend high-level rehabilitation services after inpatient
rehabilitation or transition to an assisted-living setting.
Our Interdisciplinary Rehabilitation Team Distinguished
in the community by our successful clinical and rehabilitation
outcomes, Broomall Rehabilitation and Nursing Center’s
interdisciplinary rehabilitation team includes: the patient’s
independent primary care physician, IV Certified nurses, physical
therapists, occupational therapists, speech/language pathologists,
respiratory therapists, a Certified ET/wound care nurse, social
workers, recreation therapists, restorative nursing specialists,
physiatrist services (doctor of physical medicine and rehabilitation)
and other clinical specialists. The team provides a full range of
services including: mobility and endurance recovery, communication and
cognitive augmentation, dysphasia management, orthotic fitting and
training, pain management and social, emotional and vocational support.
Patients and their families are integral members of the
interdisciplinary team, working together with the team to establish
(and re-establish as often as necessary) realistic goals and treatment
plans. Our team of professionals specialize in understanding the
individual needs of our patients and how best to adjust the pace of
rehabilitation to maximize each patient’s opportunities for a
successful recovery.
